Food Allergy Questionnaire

Hi, I am currently a student doing my A-levels. I am studying Nutrition and Food Science and as part of our A-level we have to complete a piece of coursework on a chosen topic. I have chosen to focus on the effect of food allergies on the quality of life of food allergic children and their families.

I would love to do your questionnaire, however my allergic child is still only nursery age rather than school age. I'll still fill it in if you like though.
