I'm not sure it came out all at once .. I got a call at nursery and he'd only been there a bit

It look like he had a fight with an inanimate object and the object won!

It looks like a graze to me. There's no inflammation, so it looks like a aged wound rather than a fresh one. It will heal.

It's not forceps
And no one can recall him falling at all.. Unsure what could cause this

The nursery staff need to explain it. Obviously he's bashed into something or else someone's hit him. Not sure why this is posted on Allergies though - it doesn't look like that sort of thing.
