UTI symptoms and constant pain despite antibiotics, any advice?

Has anyone suffered with UTI’s and had constant pain with it? Only relief I can get is sitting with an ice pack between my legs! Pain is in vagina and heavy pressure feeling in bladder. Absolutely exhausted as well and feel generally quite poorly.
saw the Gp who found no infection but prescribed a week of nitrofurantoin due to symptoms. 3 days in of the antibiotics and haven’t improved at all.
I’d be grateful for any advice

I think you'd expect to feel an improvement after 3 days, can you contact your GP? My DD found Nitrofurantoin ineffective too when she had a bad UTi, they prescribed her a different one which did work.
Warm baths help, cranberry juice, plenty of water...
My DD was prescribed Hiprex for repeated UTIs - it's a daily tablet that is preventative.

Are you perimenopausal? I thought I had a uti, also negative test, think now it was vaginal atrophy causing symptoms and using oestrogen cream has massively helped

How old are you. I had these symptoms turned out to be vaginal astrophy. Was put on vagifem and it worked.

Same. Years of agony until I started on estriol cream.

YourPinkShark · 10/09/2026 14:50

I’ve been using the vaginal pessaries for a week now but it hasn’t improved at all unfortunately.

The only antibiotic that worked for me after others didn’t was Augmentin. Haven’t had a UTI for a couple of years.

Endlesslywondering · 10/09/2026 14:54

Not to alarm you op, but ask them for a blood test. I had uti symptoms for a year & countless courses of antibiotics to no avail. I had pain in my bladder & it turned out to be bladder cancer.

Hope you get it sorted

Bobbybobbins · 10/09/2026 15:04

@YourPinkShark I’ve heard mixed reviews of the pessaries, would suggest trying the cream. I went through thinking pain was uti, thrush, allergy to tampons before finally realising it was the drop
in oestrogen causing it.

Mary46 · 10/09/2026 15:25

Op they desperate. Vagifem helped me had 3 utis last winter. Could not shift them. Maybe you need stronger one? Hiprex good too. Friend uses D mannose.

CoatiCutie · 10/09/2026 15:59

Try a different antibiotic, if that doesn't help then get a screening for ureaplasma (that's what mine was), also could be chronic thrush which I had to have 6 months of fluclonazole for. If none of that helps then look at a hypertonic pelvic floor with a good pelvic floor physiotherapist

I was battling constant uti type pain for a year, and then on and off pain for another year

VelociraptorsVelociRapping · 10/09/2026 16:04

If you possibly can, try to see a pelvic physiotherapist. You might be able to get a referral on the NHS but if you can afford to pay privately you could see someone almost immediately. You may have a prolapse which could be exacerbating your symptoms.

RafaistheKingofClay · 10/09/2026 16:07

You need to go back especially if you are now feeling unwell. Negative dipstick test doesn’t necessarily mean there isn’t an infection.

I’m pretty sure they should want to see you again. Untreated or ineffectivwly treated utis can turn nasty.
