Is it normal for sick leave forms to require a reason?

Do you normally have to select a reason from a list?

I've recently started a new job, and the sick leave form in the HR system has a drop-down with about 25 broad categories on it ... things like gastro-intestinal problems, back problems, respiratory problems, mental health problems, cancer, etc. I haven't seen this in previous jobs, so I'm wondering how common it is?

I've never worked anywhere where I haven't had to put a reason down.

I work in HR and the categories help us follow up properly. For example if someone says they're off with stress or depression that will automatically trigger us checking in to see if it's something related to work and if any adjustments are required. If it's back problems, we can look at their desk set up, if it's pregnancy related or related to a disability then that won't negatively impact their sickness record etc.

You don't have to put the real reason if it makes you uncomfortable. I understand the PP that didn't want to declare a miscarriage putting another generic sickness reason down, but just be aware if it's something you need support with your workplace can't assist if they don't know what's wrong.

Ours used to but now you can just put post operative or something similarly vague.

Previously you had to specify your reason, but the secretary was prone to telling others why you were off or leaving sick notes lying around in the office for anyone to see.

Our form has a free space where you can put your reason for absence. We don’t expect every detail but as others have said it can be helpful for seeing if you need extra support on your return.
