Disappearing online forms - what fresh hell is this....

I've come across this 3 times recently in relation to school or activities for the kids. Organisations that previously sent a paper form home or asked for a form attached to an email, now have a form embedded within an app. When the form is complete and submit is pressed it disappears after a brief 'form is complete' message. Although one time there was no submit button, it just ended after a yes/no selection. No receipt is given and I have no way of showing what I entered. If I click the link again I only see a blank form.

I searched online and if seems the other party must enable receipts, but there is no information to this effect as a user. I would take a screen shot if I had known. But either way, doesn't that make us very vulnerable? An organisation could put any info they want and we have nothing to show otherwise? I'm worried because the form today is important to my child and I now have to go and contact the organisation to see if they have it and I don't want to be annoying them.

Please can someone tell me if I did something wrong or is this the new normal. Also if I was to refill the form and change information, would this override the first form or would the second be rejected? What if I'm told I didn't fill out the form before the deadline, can I prove I did?

The one today was Google but from memory the last one was Microsoft, but I had same issues with both.

Always always screenshot each bit you put in.

Thanks @noclingfilm i wish I had. But even so that's not really documenting correctly as I could screenshot then change before submitting. If these forms are being used for important information surely both parties need a copy? It's madness to me that this is progress!
