Struggling with ds2's PIP form

It's got to be in next week. I'm really struggling to write much for the questions and I'm worried I'm not doing it right. It doesn't help that I'm so tired I can barely speak in coherent sentences.

Thank you. Managed to get it done and posted. Huge weight off my mind. I'm dyslexic so really struggle with forms, especially ones with a time limit.
