Anyone know where to buy "get to know you" forms for a party/event??

A while ago, I went to a hobby related event/party where most people were strangers (an evening do after a day of seminars/sessions about the hobby! The organisers handed out a sheet of paper to everyone with a list of things that you had to give to others to get them to sign their name to one of the items on the list, and you reciprocated with them.

The list items included some flirty ones like ............ would like to buy you a drink, .................. thinks you have a lovely smile, ................. likes your outfit. Most of the other questions were harmless. It was basically a kind of "ice breaker" and was actually really good fun.

A few people "paired up" for the rest of the evening, but most people just regarded it as harmless fun and it certainly got the guests socialising between them, especially good for the shy ones.

Unfortunately, I didn't keep my sheet, and I've ever since tried to find where to buy them as I think they'd be good for a hobby event evening we're planning for next year.

I don't suppose anyone else knows what I'm talking about and where to buy them??

Wouldn't it be easy enough to make your own?

Just Google some ideas for questions, type them out, maybe decorate with some pictures and print them out.

Agree with pp, why not just make one and get it printed off. It's simple to do a d there's loads of templates available.
