6 yo incontinent and secrecy, help please!

6 yo DS has been struggling with continence beginning over the past year and a half. It started with bullying at school, he was fearful and didn't want to use the loo and risk being seen/hurt by the bullies in the corridor. He started holding his wee and poo all day and would have a wet spot on his trousers when I collected him after school. The poo became so impacted that he began to have diarrhoea running past. It was heartbreaking. He would try to hide it from us, I found soiled pants in the cupboard.

We've been seen by GP and paediatrician, who have ruled out UTI, diabetes, or other issues, and also gave him two bowel cleanses and a regular does of Movicol. It appears he has had encoparaeses. He's also had several sessions with a paediatric physio who specialises in pelvic floor. He's also got ongoing therapy to support with the bullying and his self esteem.

It's generally getting better but we have weeks where he does have leaks or wet poo leaks. He's quite secretive still about his toileting so it's quite hard to know whether we need to adjsut the Movicol or if somethign else is happening. Has anyone been through this? I'm just heartbroken for him, he doesn't want to talk about it but it's also quite tiring to remind him to go change his clothes (sometimes he has a wee leak and I need to tell him to change, he pretends he doesn't notice). He was toilet trained around 2.5 years and everything was fine until the bullying when he was 4.5-5 years.
