Is this the ‘extra’ I’ve read about? How annoying when it’s so expensive! Do you all buy extra needles and extract it? Are there any downsides to that?

Yeah it's the so called golden dose but I just throw it away. Some people buy insulin needs to extract or force the pen to be able to inject it again but obviously the advice is not to do that, so its entirely up to you.

Thanks. I was wondering whether to inject it as next week I’m due to go up a dose anyway but I’m also nervous about messing about with the ‘system’!
