Absence and Bradford score....

I'd like your views on this please.
I had a long term absence three years ago when I was diagnosed with a medical condition. I ended up having over eight months off work and I had to work hard to get myself to a place where I was mentally and physically fit enough to return.
Because of the way I was treated by Occ Health and HR I have been very wary of having time off since. (It was very traumatic so I don't want to go into details.)
I had surgery late last year and needed six weeks off work to recover.
My long term absence was due to what is classed as a disability.
My question is: I'm really struggling at the moment due to my disability. I've tried very hard not to take any time off for this reason because of the whole OH / HR scenario. I've also got a couple of other things going on medically which I've been trying to ignore because of my previous attendance record - a gynae infection which I haven't sought treatment for, plus a recurrence of an autoimmune skin condition and an exacerbation of arthritis / nerve problem.
What would you do in my shoes?

I would prioritise my health.

You could take leave to get treatment for the smaller issues (normal for routine appointments) but the disability issues should be covered by your enhanced legal protection?

Speak to a relevant charity for advice perhaps.

I got home from work some time ago but have only just got moving after needing a nap!
