What do I have to do to be referred to an ENT specialist?

Since I was a child (now in my 40s) I've barely been able to breathe out of my nose at night. Sometimes during the day too. My nose looks visibly bent which doesn't bother me but must be contributing to the problem.
Everytime I go to the doctor I get prescribed stronger and stronger steroids to spray up it. I wake up multiple times in the night unable to breathe. I can't sleep if my nose is blocked.
I take strong antihistamines every night. I wash my bedding regularly.
I have a very small throat and ear canals (remarked upon by medical professionals) which may mean my nose is also very small inside.
Still my doctor won't refer me to an ENT.
Just a rant really. I'm exhausted from not sleeping.
