Chronic low back pain-spasm?

I have chronic low back pain for 6 year one sided lower right.

I'm 44 and active and lead a busy normal life and swim a lot. I'm very overweight but that is lifelong and I have considered mounjaro.

I've been through chiropractic pain clinic mri injections (medial branch was negative so had steroid)

I take amitriptiline daily snd cocodamol or tramadol for flare ups depending on how bad it is.

I had a bad flare up in January and was out of action for a 6 weeks ( I did work and basic stuff no sport or going out I was either at work or home and spent a lot of time in bed) and eventually went to a new chiropractic.

I don't "believe in chiro" but he said the joints at the sacrum were wear and tear and had a muscular spasms in lower right back.

I have done yoga and swim a lot.

I had to wait about 8 weeks to see first contact physio at GP and by then and asked for referral to MSK (as this is what the pain clinic said I could do when they discharged me) as he said they would be able to do acupuncture.

I went to the hospital for physio and it was pretty terrible. Anyway I got some basic yoga exercise todo and they are fine (cat cow, bridge,
knee to chest and child pose)

I'm thinking that this pain is not neuro or bone but actually just a chroniclly spasmed muscle. So would a short course of diazepam help?

What else might help. Would some massage from a private physio be better

I'm fed up of jumping though NHS hoops.
