I can see how spreading butter on toast could sound "scrapey."
I found the bath towel joke funny. I didn't quite understand the two mile long neck joke, but it might be lost in translation.

I guess because it’s hard to swallow so you have to kind of extend your neck to get it down … ? Need more heartwarming stories like these!

Oh yes, probably that. I watched the video and he toasted his bread before making a smoked salmon and avocado sandwich. They might not have realised it had been toasted and been confused about why the butter being spread was making a rasping sound and thought it was weird bone dry bread. 😁
