Can you get a medical history from the GP?

Sorry if I've worded that wrong! Basically, I'm applying for PIP and I have a long history of visits to the GP since I was about 15 discussing my mental health. However, since I was 21 and moved away, I haven't had a constant GP, just whoever was given my appointment.

So, I don't have a particular GP who knows me well. The one I saw often as a teenager doesn't practice anymore, I believe.

But, surely there is a record somewhere of all my visits and diagnosises and discussions etc? And if so, how do I get it?

My NHS app used to have history back to childhood, but my surgery changed providerand now it only goes back to when I registered three months ago. You might be lucky

Go into your surgery and request a SARs form. You tell them what you want snd they have 28 days to give you the info.

It's part of my job.

Actually often you don't need to request via SARS. A brief summary print out would list your current health problems and recent appointments. This may be enough for PIP and is usually quick.

Yes, ask if you can have a printed patient summary with all your medical conditions and medications listed. They may be able to supply via email as a pdf.

So I've got on the NHS app and it has all the jabs I've had since a baby, and has a history of all the medication I've taken, but nothing about what conditions I have and what I've been diagnosed with.

you don’t need this as evidence …..have you got copies of clinic letters from your mental health team…..they would do
