Permission for child to travel abroad

Planning on taking DS (14) to NY in December, I've already taken him to Aus, Europe, Singapore etc. but the US has been the only one where it specifically states in the entry requirements you need a letter of consent from the other parent.

I should be able to get permission - exH had no issue with DS getting a passport and hasn't seen DS since he was 5. however, I know he won't put his address/contact info on the letter (all I have is an email address for him, he doesn't want me to know he left the country even though my family regularly see him back in our home country), and he won't go get it notarised as that's extra effort for him he's just not going to do, is this going to be an issue?

Do you and DS have the same surname?

No, I use my maiden name, he has his dads. My surname on the birth certificate is my married name if that makes a difference
