Ds, 19, Myocarditis, 3.2k Tryponin, ' MRI & send home'

Follow onto previous thread: 'Ds, 19, what are these symptoms' (don't know how to link, would be happy if anyone could do it for me?)

Ds in hospital since Sunday 1am. Presented at A&E with severe chest pain, vomiting, sore neck & shoulders. 8m after a bad dose of covid which had left him exhausted & with occasional 'stitch' feeling in chest during rest.
Bloods showed a Tryponin level of 2.5k, which increased to 3.7k on Mon, 4.2k Tues, 5.5k Weds. Thurs down to 3.2k.
Echo scan showed moderate LVSD. Given bisopralol. Re-scanned yesterday & told now normal. LVSD dx retracted. Still has dx of Myocarditis. No cause?

MRI scheduled for this afternoon. Results 'take 2 weeks'.
Suggest go home meantime.
He's currently on high dependency bed but they wants to take telemetry off & walk down to cafe. I asked what sort of activity level next 2 weeks - 'normal' - 'go on holiday if you like'. I mentioned we have a holiday to Malta booked next Weds & Consultant said: 'up to you'.

REALLY ??

If you are worried then call the hospital "call 4 concern" service, it might also be called Martha's rule

Could you ask for a second opinion?
If i remember correctly (it has been 25+ years since I studied this) Trop T can be raised due to muscle damage too, I remember a guy who had completed a marathon and was in early acute renal failure who had a massively elevated Trop T. It doesn’t have to be cardiac related, even though the most common ones seen usually are. I’m sure you’ve had great advice though, not seen the other thread. Good luck to your son x

Most myocarditis in teens/children is viral and gets better on its own without any treatment.

So they will want to get him up and about and if he's OK, get him home.

Waiting and resting in the UK or in Malta isn't going to alter the course of his recovery so in a way the consultant was right.

I'd check with your travel insurance though. Hopefully if you had insurance prior to any of these symptoms they'll either refund you the trip if advised by insurance company not to travel or cover him if he hadn't been told not to travel. My concern would be anything, even something unrelated, happening while away meaning that you find he's not covered and ending up with a huge bill if they haven't been informed.

How is your son feeling? Have you had a discharge notice? That should detail ant follow up.

IME, MRI results usually take 2 weeks for analyse and writing the report but if the MRI shows something serious it will be expedited.

My ds had pericarditis after COVID vaccine, not as serious a condition obviously.

He had a follow up with cardiologist a few weeks later to check all resolved and then discharged NFA.

Make sure you have excellent travel insurance if you decide to go abroad.

Gosh I definitely wouldn't travel.

Going home seems to make sense if it's viral and can be improved over time with rest

I dont think he’d be able to fly tbh but I would be getting clarification from another cardiologist if possible
it’s mostly likely post viral , I had myocarditis after covid and now have left ventricle hypertrophy but my cardiologist assures me my heart is pumping well .
Resting is the best way to recover but it may be an idea to see if he can be refered for cardia rehabilitation

The holiday (I booked, with insurance, on Sat teatime & A&E admission was 7 hours later at 2am this Sunday).
Don't mean to focus on holiday esp. I think I'm just struggling to get head around speed of change & holiday feels extremely different to 'heart failure' which I was still being told 48 hrs ago? (esp if flight might put strain on heart / DVT risk, heat & tiredness on Malta)

Should his Tryponin be at a particular level, or just 'dropping' for discharge?

Wondering about his Uni place too. He's already pretty stressed about it.

This happened to me last year. They thought was heart attack but mri showed no heart damage so the eventual dx was myocarditis/pericarditis.

So awful at such a young age of 19.

He had covid jabs at rhe sane times as most of rhe population yes. A mix of Astrazenica & Pfizer, I think. Don't remember any particular reactions.

Please, if in all of this you can, follow up with the earlier suggestions about speaking to, and writing to, PALS about the cancellation of treatments etc. due to conflicting information and inappropriate acceptance of ASD as an element of your DS's reactions. Don't know about where you are, but usually there is someone from PALS available at various times in the hospital I would go to so they may not be too far away.
