Picking up a prescription from a different trust

Would there ever be a situation where you would have an appointment at one hospital and then pick up medication from another 45 miles away? One hospital is near somebody's home, the other their work.

Depends on the prescription. If it's a hospital pharmacy prescription then you have to pick it up from the hospital own pharmacy but if it's an electronic prescription eg by a GP you can have it sent anywhere.
