Advanced Care Directive

My father is very ill in hospital .The doctors keep asking if we have any "advanced care directives". They are things like no CPR or intubation. He is 86yo and has dementia and now sepsis.
We said No, we don't have one.
I spoke quietly to my mum and said I was surprised Dad hadn't sorted something and she said "He did, it's at home". I asked where and she said she won't tell me and she is going to rip it up!
My mum is seriously the nicest person in the world , but glued to my father for almost 70 years and I don't think she can face life without him.
SHe is 89 years old and suffering with a different form of dementia herself. They have lived together in their own house until this week. I feel I am going to have to be the bad guy to tell them they can't go back. I've been saying is for several years. My sibling disagrees. Feck
I have no idea WTF to do.
Very sad.
Any ideas?

The Advance Directive should be lodged with the GP, so check if they have a copy.
