If prostate pain isn't an infection what could be the issue

My partner is getting results back tomorrow regarding his prostate. An infection has been ruled out which makes me think it's going to be bad news. What could it be if not infection?

Benign enlargement? I don't think there is pain with cancer that is the biggest problem, usually pain starts when it's spread outside the prostate. Bone pain if it's really bad. My DH has prostate cancer, well controlled for a good number of years now, so don't panic too much if that is suspected. He had no pain, but other more subtle symptoms, marginally high PSA, cancer was confirmed by (second) biopsy.

It could be prostatitis.
