Help! Changing GP when I move

I have a diagnosis of ASHD and depression for which I get monthly medication on the NHS.
I've moved out of a "generous" GP area into another borough completely. So my old borough still had ADHD diagnostic services for adults 3 years ago, and my new area didn't. So if I'd lived in my new area I would not have been eligible for nhs diagnosis and treatment for adhd.

Will I take my diagnoses with me and my prescription "rights" to
Medication?

Am panicking at the idea of having to start again

If you already have a diagnosis and are currently prescribed medication then your new surgery can just carry on prescribing. They don’t get to cherry pick which conditions you’ve been diagnosed with, your notes go over to your new GP detailing all your conditions & medications, although it can sometimes take a few months to catch up. Take a copy of your repeat prescription with you when you fill in the paperwork at your new surgery.
