Accessing medical records.

Sorry if this is the wrong topic, I didn't know where to post...

Does anyone know how to access medical records?
I've already been to my GP and asked for a copy and I received a very basic print out that is missing alot of things so I can only assume I've asked for the wrong thing.

I'm specifically after all details held on 2 operations I had in the last 8 years. I want to know the full findings following an exploratory op, and what was done during a 2nd Op as well as what exactly was diagnosed. Would it be the hospital I had the ops at that I need to ask rather than my GP surgery?

I would request a subject access request at the hospital you had your operations at - they'll likely have more information than your GP.

The operation stuff should be with the hospital, I have the letters from the consultant to the GP but no details of the operation itself. There isn't one big medical record with everything in it, they are all held in different areas on different systems.

Agree you need to make a DSAR request from the hospital. The process and form should be on the relevant website or speak to PALS.
