How to approach the nursery?

My child is 3.5 years old and he started attending preschool sessions for two days per week at local nursery. Overall he is happy and I am satisfied but on few days when I pick him up he does not look very happy and confident as if he is left unattended I am not sure about the reason. Last Monday when I went to pick him up he was sitting by the side of key worker and she told me while sitting there that another young child had an argument with him and poked the pencil into his eye. He got two scratches but fortunately eye was saved. He was given cuddles and area cleaned. Because of crying he had runny nose for which he was not wiped nor he was given any thing to wipe his nose that when I came to pick him up his nose was dried and dirty they did not even mind to clean it. They gave me a form on the application for acknowledgement of the accident. When my husband looked at his eye he was furious regarding safety of the child and wants to remove him from the nursery and give bad review. Ideally a young child must not be given pencil while unattended he could have caused permanent damage to my child's eye. Please I want an advice regarding this issue. Should I talk to the nursery? In which way? Or I should remove my child from the nursery.

Your poor little boy!

I would ask for a meeting with the manager to discuss how they might mitigate this happening again in the future. Express your concerns about the child having a pencil and that it could've been much worse.

No idea if the above is reasonable or not, but I would feel really sad for my DC if it happened to them.

Children will have access to pencils, scissors etc at nursery and there will not be an adult hovering over every child as they use them.
