Did you tell the receptionist that you're using 4 pumps a day? How many tubes did you get in August?

You might have to make a GP appointment (for today) and get a prescription. It's worth being assertive because you need it today.

Did the GP indicate the right amount on the px? I collected my last HRT bag to find 3x packets of Utrogestan (3 months) but only one canister of Oestrogel (36 days). I phoned the surgery and the GP issued the other 2 Oestrogels same day.

Sounds like the receptionist is massively overstepping there. It's not down to her if you need more. Can you use e-consult or your nhs app to order more? Or phone again and ask for a phone consultation?

On another note, I started on the max dosage but I'm down to one squirt a day now so hopefully you'll find as your oestrogen levels stabilise you won't need so much. It's bloody annoying that the prescription is for two months at a time though - I actually thought that had changed to six months at a time earlier this year.

You need to speak to a GP.

As a prescriber, it's likely they have just ticked one tube per month without doing a calculation of how quickly 4 pumps a day would run out - very easy to do on electronic prescribing.

Receptionist is rightly telling you that you can't order again but unaware that the prescription was for the incorrect amount in the first place.
