NHS app notes are not all mine

Recent phone call with a paramedic based at my GP surgery was written up on my notes but directly after these a set of notes that were not mine appeared. I contacted the surgery and they said they removed them the
actual note was removed and i received an apology but just looked and a similar shortened version is still there, its not to do with me. I tried again letting surgery know through their online system but have heard nothing in over a week and its still there should i be concerned?

Also the original text that was not related to me was removed but a second entry with half of the information then appeared so seems they can be altered.

They can remove things- I had two incorrect diagnoses on my notes that I saw on the screen by chance. They’ve been removed

What "screen"?
How did you see them "by chance".
Seems like there is a Data Protection issue here.
If you saw them "by chance" then so could someone else.
IMO I would look into this.

NHS app gives you access to your medical records so i often check if i have seen anyone or am waiting on test results, i also use it to order repeat prescriptions. Think i will try going in to ask its just annoying that their online contact only works sporadically.

How is it ‘no big deal’ that someone might see another person’s confidential medical information, @Ladyoftheknight?

First time it happened i went in then after a week when they were still there i filled out the klinik online form, got a message the next day apologising saying it had been removed, then checked and original entry was gone but a shorter one had appeared. Tried the klinik form again but have heard nothing in a week so will call in tomorrow.

Hmm seems a big deal to me to be honest if my records are appearing on someone else's record and vice versa. Those records are used to support things i might be diagnosed with and if the wrong info is on there then it could have health implications same for the person whose notes are on my record instead of their own surely.

IMO I would look into this.

The screen in the GP consulting room- definitely my notes! One of the things was a very serious life changing diagnosis- the GP looked into it and it had been put on my notes by mistake (and I assumed missed off the other person’s notes).

That sounds awful, at least the notes on mine were not particularly serious but it does worry me that it happens.

Can you threaten to report them to the ICO? (Information Commission - deals with serious data protection issues). They have the power to levy huge fines on organisations that don’t comply. Also get the CQC on to it.

Just to add, one local government organisation was fined to the eyeballs not long ago for not doing a proper clear out of old files from a Children’s Services office. This might merit similar treatment.

Thanks, good plan as the more i think about it the more unacceptable it seems.

Update to say it has all been resolved, no explanation for why it happened, just seems to be some sort of glitch as they swapped to systm one. Annoying as most of my previous records are no longer visible although they say they will eventually come back.
