Starting to fill in a DLA form - I have a stupid question!

I’m assuming that I ignore this page as it doesn’t apply?

(My brain stops working when filling in forms, so just want to double check that I’m not screwing it all up!)

Next stupid question:
Ds has to have more regular eye tests due to a cyst on his eye - it doesn’t affect him, should I still put it down?

You need to answer questions 11 and 12 then go onto questions 15 assuming that is also no into question 17 .

I need to go through this process myself for my child .

Put the cyst down. Put everything medical down even if you don’t think it’s relevant.

I imagine if you go back on the form it might tell you to skip those pages if UK national/always lived in the UK.

I assume you tick yes to question 11.

Put every thing down that you can medically prove. It's up to them to decide if it's relevant. Even if the cyst doesn't bother him he is still having extra appointments because of it (so needing taxi fare or petrol, or time off etc).

My surgery won’t do that unfortunately.
They've suggested CAB, but this is beyond me right now.
I have done this before, not that anyone would guess!

Each question is separate. You asked if the page should be ignored. No, do not ignore all the questions as some will apply to you.

Unless you meant to type Q10 instead of page? In which case I would put a line through it and write N/A, then go on to Q11.
