Passport rules?

So I have had a adult passport before but it's expired, I also need to change my name to my married one, do I need it countersigned?

Struggling to understand what this actually means as well:

renewal of a passport if your appearance has changed and you cannot be recognised from your existing passport

Like yes I have aged and now have long hair rather than short in my old passport photo but does that mean it needs to be counter signed?

I recently got a new passport under my married name for the first time and it didn’t need to be countersigned - they just needed a copy of our marriage certificate.

Is your face still recognisable compared to the last photo? Hair length is irrelevant, as is natural aging in ten years for everyone.

Thank you @Upandonward I still think I look like my passport photo even though it's 12 years since it!
