Under active thyroid, on medication but just cannot lose weight

I am so depressed. I was diagnosed with an under active thyroid approx 18 months ago after going to the doctor for extreme fatigue, joint pain and the fact I was putting on weight despite exercise and decent diet and not changing much in my life.

sinde then I have had 2 follow up blood tests which deem my medication is at a correct dose: 50micrograms of levothyroxine.

I have not lost a pound. In fact, I seem to be putting on weight. I am so fed up. I am on my feet all day, walk lots, do 4 x hiit/spin/swim sessions a week.

I eat well, I am careful. I am getting desperate now. Surely if I’m on the right dose of medication it should be just as easy/ hard for me to lose weight as the next person? Or will the fact I’ve got an under active thyroid make is more difficult, even with medication.

please tell me your experiences

@Decafflatteplease I’ve also wondered whether I should take Vitamin D but my doctor hasn’t mentioned it. Perhaps I should just do it.

I have hashimotos disease and take 125mg of thyroxine a day

I'm very very slowly losing weight using ww but plan to speak to my gp. I watch what I eat and do PT 3 times a week

No point getting pushier with GP, you'll alienate them without getting any better help.

Do your own online research, there are some good websites and some good facebook groups. Once you have a better understanding you will be better placed moving forward. I supplement the Thyroxine (T4) prescribed with T3 (Tyromel) that I buy in Turkey. (The NHS rarely approves T3 as a months supply costs them a fortune due to their outdated and stupid contracts. (AT best £63 per month down from at least 4 times that)). When I buy it in Turkey it costs me just over £1 for 3 months supply - online it is currently about £11 for the same so Fuck knows why it costs the NHS so much.

But I am not advising you to do what I do. The advice I am giving is to do your research and then decide what to do.

Hi Nanny,

Where do you buy your T3 from if you don't mind me asking ? I also buy from turkey but pay a lot more than that! Thanks

I have hasimoto and manage to lose weight by taking T3(also buy tiromel from.turkey). Also I do low carb. The only diet that I can lose weight on. Calorie control doesn't work for me.

I'm not sure about the online price. The £11 was what I saw in a quick search.

I go to Turkey on holiday and buy it whilst there. From any pharmacy. I've used a few in Hisaronu and Olu Deniz. All about the same price. I normally get 5 packs at a time which last me about 16 months. Last time that cost me £5.75 ish. It's perfectly legal to buy them over there and bring them back for your own use but not to sell. In the past I have had friends bring me some back when I haven't been going myself.

I ran out over lockdown and put 20lbs on. So I now have about 2.5 years supply.

I have Hashimoto's and take alternate 75/100 mcg.

I've just started with MyFitnessPal and the main thing I've learned is that thyroid or no thyroid, I wasn't losing weight because I was eating and drinking too much. I need to get below 1200 a day - or exercise more- to lose weight.

Keeping below 1200 will necessitate a serious change to my dietary habits that I'm not very happy about :(

Vit d - my sister had thyroid cancer and her endo advised she take the spray d3 vit d spray and for me too (I have congenital hypothyroidism and have no thyroid either) def makes a difference if I forget to take it.
I’m on quite a high dose of thyroxine (250mcg) need a blood test soon really.
am the biggest I’ve ever been and feel crap it’s just not for shifting

TheOtherHotstepper · 23/01/2023 16:10

For anyone without a thyroid, the rule of thumb for calculating dose of thyroxine is 1.6mcg per kilo of body weight.

Some guidelines now recommend this calculation to work out your starting dose if you have a thyroid, but it is underactive. Try it. If nothing else it might give you an idea of what to aim for.

Great thank you. Going to turkey in April so will make sure I stock up!

26 years on levothyroxine 200 per day. Never lost a pound unless on v calorie restricted diet. I.e less than 1100 a day.

PrincessHoneysuckle · 24/01/2023 22:40

My thyroid is only "slightly" out and I've been on 125mcg for 9 years

Siean · 28/06/2025 09:32

Hi NannyGythaOgg how did you work out how much TR Tiromel to take? My TSH has come back at 10 😭 when intake levothyroxine it plays havoc with my psoriasis. My weight gain has been 10kg plus a year minimum and its stressing me out . I clean eat and weight train twice a week and weight just keeps increasing.
