Doing this on GOV website so please only comment if you know how to do this part-

All numbers helplines are closed and I have to send this off first thing Monday morning

I am changing my surname

• current name

Abbie Max Weeks

• New name

Abbie Max Meadow

(That's not my real name) but I need to know what to write and what to tick.

Tick surname for the first and put 'Meadows'
And tick surname for the second and put
'Weeks' obviously without speech marks
