What info do i need to include in an e consult email?

I want to send an e consult about my 17 year old daughter - i know shes 17 but she is autistic and she relies on me for this sort of thing still.

She has only ever been to the gp about 3 times - last time was in august and they told me off for not including enough info in econsult form so im hoping someone can help me here because between me and my daughter we are pretty useless at this!

So she has been freakishly healthy until last year when she got covid in august - she got pnumonia following which she had antibiotics for it and since then she has had chest problems on and off. She has been wheezy alot and has had an almost constant cold.

She is pretty bad at the moment and i want the gp do check her out to make sure there is notbing more wrong.

So she has no temp.

But her breathing is loud and you can hear her chest whistling.

Her apple watch has given her warnings about high heart rate at rest today.

I checked her oxygen on a finger thing(we gpt this when she had covid) and it was 96% ( which i think is fine?

Her breathing is at around 23/24 per minute. This is what i should have included last time apparently.

Is this everything i need to include ? Do i need to add more?

Also has anyone had this after covid especially younger people?

Based on the fact she has an audible wheeze, increased pulse and resp rate I'd call 111 also for advice. Sats are okay but we don't know her baseline and if they've dropped.
