5 months ago I submitted a complaint to an organisation on behalf of my daughter in relation to safeguarding, GDPR, and other issues amounting to misconduct from the allocated worker. The organisations own complaint procedure wasn’t followed and my daughter not spoken with, nor was all of the information provided as part of the DSAR despite the organisation confirming there was no exemptions applied.

Approximately half of the concerns raised were upheld. However, there were countless factual inaccuracies in the outcome report that mean the several of the remaining concerns raised were not properly investigated or simply not addressed at all. I raised this within 24 hours of receiving the outcome with factual evidence but they have refused to review or amend the inaccurate report.

The report focuses heavily on the feelings of the worker and not my daughter, understandable since they didn’t speak to her or contact her in any way. All they have offered is a mediation session with an external provider that they appoint and meet with prior however this still won’t change the non - factual report. I am not sure what to do next, any advice would be great fully appreciated.
