DS has just been diagnosed with autism however in the report it has incorrect information saying is dad lives with us which he doesn’t- it seems they have used his old report from 2018 to reference - can this be changed as it’s incorrect

Write to them and ask for it to be updated?

Does it really matter? I imagine you have been on this path for some time and that takes its toll emotionally.

That’s the thing the report has literally just been written on Tuesday so can it be rewritten? Or just that sentence taking out- it might not matter to some and maybe I am being emotional but I feel it’s an important document that needs to reflect truth and not inaccurate assumptions

I work in a similar service. Definitely if you contact them about it and ask it to be changed, they can amend the report and resend to all parties.
