Name change from maidenname-husbandname to just maiden name (not getting divorced!)

I got married 6 years ago and changed my name via deed poll to maidenname-husbandsname

I hate having a double barrelled name and wish to revert back to maidenname only (we are not getting divorced!!)

How do I do this - do I need another deed poll?

My passport is still in maidenname only so that should be fine but I have no clue how I go about changing bank details, doctors records! Has anyone else done this?

Why don't you just pick up the phone and inform them that you require them to change their records to show that from a specific date set by you, you will be known as Ms ..................
