She wants to know if you look at a message multiple times, whether the sender would know??

No they won’t know, it just shows as read, the first time you read it and nothing more the following times x

But if they happened to be online at the same time they could see she was too. She could change her settings so the sender doesn't know she's read the message or when she was last online though

No, and you can switch off read receipts if you don't want the sender to know that you've read a message (I think it also means you can't see if your message has been read).

But there's no way to know what you're looking at when online. However it does show if you're typing to a particular person if they are online at the same time.
