Help - vaccination certificate in married name

I have my maiden name on my passport, credit cards and email address. I have my married name on my driving licence, doctors and utility bills. No reason for this other than I started getting pangs of sadness at changing my name so didn’t change everything over.

Now I have booked a holiday abroad. My passport is in my maiden name but my Vaccination Certificate (to confirm I’m double jabbed) is in married name.

I asked the Drs to change my name back to maiden name, but they refused. Does anyone have any advice- it’s not as simple as changing the name on my passport as tickets are booked in maiden name.

I created a pdf document that combines my marriage certificate with the vaccination certificate and uploaded that to all the passenger locator forms. That seemed to work.

It's not guaranteed of course.

I have since applied for online and received a Swiss vaccination passport that shows my maiden name using the method described above. It was SFR 30 (just short of £30) and is valid for a year. I did this when visiting Switzerland, not sure if that works if you don't.

I had to get my doctors to change my name back. It took 24 hours or so and then the vaccine passport updated to match my documents.

I’m not sure the doctor can refuse, to be honest…
