Mental health disclosure/ freelancing

I have chronic depression and anxiety, which I manage with medications and lifestyle changes (exercise, healthy eating, no alcohol). I also only work part time, freelance, which suits me as I can rest when I need to etc.

The depression and anxiety have been getting worse for about a year. I put it down to normal Covid stress, but it;s also been difficult as I haven't been able to get a doctor's appointment or talk to anyone about changes in medication etc. It's had a huge impact on work - as well as panic attacks, which leave me exhausted, I've been forgetful, slow, unfocused.

My question is: I finished a freelance contract in June. There was some admin I should have done to finish it off but I completely and totally forgot. I want to get in contact with my (old) boss, ask him if he still needs this to be done and do it for him.

I want to say in a very simple and professional way - I've been ill, and I dropped the ball on this, and I'm very sorry but will now try to rectify my mistake.

Should I mention the fact that I've been struggling with my mental health? Or just leave it vague? What if he asks?

That's perfect - no need to explain further. Most people will just say "I'm sorry (to hear that), I hope you're better now" rather than ask for details.
