Has anyone flown to the Azores recently? Testing Question

After looking at all both government websites I am so confused.

Do you need a negative PCR test before you fly to The Azores if you have a vaccination certificate? The information is so conflicting!

Is the questionnaire thing on its own okay? That's what I get sent to when looking for a Passenger Locator Form.

Help! We fly Saturday.

They are off the coast of Portugal, about 1500km into the Atlantic.

From what I can see is, the worst case scenario is, we get tested at the airport (which is fine) but would really appreciate the experiences of others

Entering the Azores
Before you travel to the Azores
All travellers, including children, must complete and submit a passenger questionnaire.

On arrival in the Azores
With the exception of children under the age of 12 (up to and including 11), all travellers must:

take an RT-PCR test, or
show a negative RT-PCR test taken within 72 hours of departure, or
show their EU digital vaccination or recovery certificate
Check the regional government’s website for full details of entry requirements.

From the government website. Yes, you’ll need a PCR for the Azores and fill in a passenger questionnaire.
