Greece - Passenger Location Form

We are flying to Rhodes in a week and have been told we need to complete a PLF. According to the website, you only need one per family, however, it only seems to allow you to tick one box for vaccine/covid test, but myself and dh have had the former and our 2 DS will have the latter. Anyone know how to get round it?

I don't think you have to. Just tick what the adults have had (ie the vaccine)

Yes, I spent a lot of last night trying to work this out, but lots of people on TripAdvisor very clear it's only the first adult whose vaccine info goes on. Everyone is checked on entry anyway.

If you happen to work out how to store it in the phone app please post tho!

we went to Greece earlier this month
Best thing we did was print out or have hard copy of vaccine certificates
and also paper copies of PLF forms.
They looked at them on check in and again at passport control in Greece

So many people were trying to use crappy airport wifi or the phone companies overwhelmed services to locate forms
if you want them on your phones download , save and put into files that dont need wifi

returning the UK plf is one per person though children can go on parents forms
