Saying one thing. Doing another. Is this a sign of anything?

So my husband is starting to do this.

This weekends examples:

We had a conversation about shopping. Decision made we would go to Aldi for main shop as (his words) the fruit is better there and then pop into Asda for some sushi and anything else we can't get at Aldi He drove straight to Asda and when I commented said I thought you wanted Asda for sushi and we can get everything here.

BBQ yesterday - decided on sausages. He took out pork.

Most awkward one...we are helping some people with planning and a mix up with what he told me one guy had said could have meant it went it wrong! Luckily I thought to double check (partly because of these other things) and he had got the message wrong.

No harm done on any of these things but is it a sign of anything more than not concentrating?

Poor hearing perhaps?
