When do I tell work/colleagues that I'm leaving?

I have received a written job offer which I have accepted, subject to pre-employment checks.

My notice period is only a week, but I will need to tell my manager in advance as I need a reference from her.

Should I just ask for a quiet word with my manager and mention it (and ask her to be a reference) before I go to HR?

I've never resigned from a job before!

It depends on your company policy. Some companies prefer it if all references came from HR. I don’t think you need your manager’s permission to list him/her on your CV. I know at my old work place, any reference request sent to my manager would be forwarded to HR.

Unless you think there’s going to be something wrong with your reference, I would give notice without waiting for your reference to be checked, that’s between your new company and your old company. If you feel like it, you might want to have an informal chat with your manager before handing your official letter of resignation.

It might be a good idea to talk to your new company about ideal start date and then include that in your resignation letter. It might result in longer than one week notice or alternatively you might want a break before starting your new job.
