Do I opt out (NHS opt out type 1) for my 17 year old or does she do it herself?

Does anyone know if I fill in the NHS Opt Out 1 form for my 17 year old or if she fills it in herself?

It says that I am supposed to fill a form for each of my children but I don’t know if she counts as a child given that she is over 16 and could technically register at a GP surgery by herself.

If you look online it says you can do it from age 13.

Age 13 is for Opt Out 2 only I think.

The Opt Out 1 form only specifies that you are the parent or guardian of the “child”, but says nothing about age.
