It means they have accessed or been referred to MH services extensively.

It's fairly unambiguous.

I suppose there is an undertone of 'for all the good it did' depending on who's saying it.

It's a nicer way of saying they've got a long history using those services.

It doesn’t mean anything bad, it just beens they have been under services before. I’m a care coordinator, don’t worry Smile

Long complicated history, lots of interventions, possibly not all successful. Agree with @lughnasadh there can be a certain connotation associated with it sadly but not necessarily always.
