2nd Vaccine after 12 weeks

I was supposed to have my 2nd vaccine this weekend but the vaccination centre is being shut after tomorrow. It is being moved to another location and my appointment has been rescheduled. However, my new appointment would put me at 16 weeks since my first vaccine. My employer is saying it doesn't matter that it's out with the 12 weeks, is this correct?

I thought 12 weeks was the latest you could have it. Can you reschedule online?
