Any dog owners know about how spots?

My 16yr old dog gets occasional hotspots when his fur gets long. Haven't been able to take him to the groomers so he is now licking a spot on his hip which will develop into a hotspot.
I have Isaderm for this and applying it will stop it but I have run out.
As I had it more than 12 months ago the vet won't repeat the prescription without a consultation which is £30.
This seems overkill as I would just be showing the vet a wet patch of fur which will eventually turn into a hotspot. They say it's to make sure that he is well enough to be prescribed Isaderm but then they offered me a telephone consultation for £30.
So can anyone recommend me something off the shelf for hotspots? I don't want to spend nearly £50.
