Dsis care plan states “o has an undiagnosed condition”the name of the condition is stated if this condition isn’t diagnosed how can it say she has it? Can anyone confirm what this sort of wording tends to mean?

I assume it means she is showing signs/behaviours of an illness but they have not been able to define which illness or condition it is yet.
