Lost my ability to talk to (most) people

Sorry, this sounds really stupid, but I have literally just realised I cannot speak with people outside of my family - it's been pretty sudden, but I am not really sure where it started. I sort of realised I was switching to typing comments on zoom rather than speaking as it felt like something was getting stuck when I tried to speak on zoom group video calls, but I thought it was done to the group associated with that call.

I have now realised I have the same feeling when I tried to speak to a shop keeper (twice).

Freaking out a bit - has anyone been through anything similar? I am fine talking to myself and my family.

Is it an actual physical thing, OP or a psychological one?

I think it must be psychological as I can speak to my relatives. I am already a patient in a specialist trauma clinic, but I have never had anything like this.
