Covid numbers please explain

So I have read that Sheffield is to face localised measures because numbers have reached 3 in every 1000 of population testing positive.

Please help me understand.

Is this an estimate of the overall population, most of whom will not have been tested
OR-

Is this based on all the tests in Sheffield, so out of everyone tested, 3 in every 1000 have been positive?
