Has anyone changed their child's name? (By deed poll)

Is it as simple as- download a form, fill out and send off to the appropriate address?

All parties with PR are in agreement, although I am aware I will need to include a letter of written consent from my sons father (who I am no longer in a relationship with).

Is it all fairly straightforward?

I did it for my son but it was seven years ago. All we did was fill in the form (very simple), get it signed by a witness (we just used a family friend) and then send it off. It was really easy

You'll need to get it enrolled at Court to make it official so that the passport office will believe it, I think, but apart from that, it's fairly straightforward
