What to say to gp for sick note

After months of being bullied at work, I've finally reported it. The thought of going back in while it's investigated is making me so sick with nerves, I just can't face it. What do I say to the gp, just literally what I've said here? Can i just ask for a note?

I told mine I was really struggling, anxious about things, not concentrating properly, feeling constantly on edge, stressed about how things were panning out and feeling really low. He signed me off for the rest of the week.

I said to the GP basically what you said here, then burst in to tears like a snotty toddler and she signed me off then and there

Thank you. I'm a bag of nerves about everything at the moment and going to the gp is stressing me out even more.

Same as Shirley I burst into floods of tears she ha r me anti-depressants and two weeks off. I could have taken noes but I was worried about it going on a reference (stupid really). I was in such a sobbing state that the pharmacist hugged me and told me she knew what I was going through.
