Public transport rudeness

Fucking hell, why is there no courtesy on public transport and what can we DO about it?
I try to be all "after you", letting people on the train first, offering others the seats, etc., and essentially it just means you get trampled. I can feel myself losing the will to do it, too. What can we do? Are we teaching our kids transport (and other) manners any more? Or is this just an anachronism?

I live in London and apart from the odd wanker we all rub along on PT just fine.

If someone becomes Ill or needs help in any way, someone will raise the alarm. Otherwise people ignore each other.

TfL do the occasional courtesy ad campaign but that's it. I teach my daughter courtesy and which side to stand on an escalator and to help others who need it...and how to say to the inconsiderate wankers to reign it in.
