Dentist, is this normal?

I go to an NHS dentist.
Seem to see different dentists each time I go. None of them will do a scale and polish, you have to see (privately pay £60) the hygienists. They state no proof that that this is useful procedure
This wouldn't be so bad but they haven't had a hygienists in post for 12 months until a week or 2 ago.
I saw dentist about 8 months ago, told all was fine despite asking questions. (My teeth are a bit crooked but I have no fillings, mid 40s)
Since then I've had to have a wisdom tooth removed (privately from an out of hours dentist, infected and impacted).
Saw hygenist today who was very blunt and told me I have gum disease, stage 4 in places. That I should have known this as was picked up during previous appointments.
I'm quite upset. I'm worried I'm going to loose teeth.
I don't know if to remain with this dentist or pay to go fully privately (there aren't any dentists taking on NHS patients where I live).
Sorry for ramble but this has really got me down
Now worried I have really bad breath and am heading for dentures.
Any advice gratefully received.
